Well Child Care Appointments
Routine well child appointments are a vital part of your child’s healthy growth and development.  You probably have many questions about behavior, development, nutrition, growth and parenting that don't require an immediate visit with your child's provider, but all your questions are important!  As questions come up please write them down!  You can review this list with your child's pediatrician at the well child check up and get all your questions answered. 

 

 
To determine when it's time for your child's appointment, our providers use the following recommended schedule for well child appointments .

Appointments for well child care fill up in advance, so please plan ahead.  As a convenience, you can request a well child appointment with your provider online or by calling our office.

While insurance plans often cover routine well child appointments, we suggest you check with your insurance company to confirm your coverage.

School, Sports and Camp Forms

If your child needs camp, school or sports forms completed for participation we can help.  If there is a form provided by the organization please obtain it, also some school forms can be found here on our forms page.   After you have completed the parent section, simply drop the form off at our office and we will complete our part for you.  If your child has not been seen within the time specified on the form, a provider will need to see your child to be certain that all health conditions are addressed as required to complete the form.  Please call our office for an appointment.

Immunizations

Immunizations are a vital part of medical care.  We follow the American Academy of Pediatrics immunization schedule.  There is flexibility in the schedule and your provider may make changes as your child’s situation dictates.

For children who are not insured, our clinic provides vaccines free of charge through the Vaccines for Children Program.
