Payments

There are three ways to make payments to your account:

1. Online through our
   secure online bill payment site.
   
2. Through the mail, send payments to

3. Come in to either of our office locations.

Insurance co-payments are due at each visit. If you do not pay your copay at the time of service there will be a $10.00 processing charge added to your account. Please read our Financial Policy for additional information.

Bring Your Insurance Card to Every Appointment

Please bring your insurance card to every appointment. We are happy to bill your current insurance and we will ask for this information at every visit.  We need your current insurance information to avoid delays in payment.

Your Health Insurance Plan

While we are contracted with most plans, it is important that you check with your insurance plan to ensure we are a contracted provider.  Please select the following link to see if we currently contract with your plan: list of insurance plans we accept. We will help you in any way we can by billing your insurance, however, only your insurance company will be able to tell you the specifics of what your plan covers including: copayments, well care, referrals and prescription coverage.
